Liability Release Form
In order to use the skatepark, a Liability Release Form (waiver) must be filled out (download available at bottom of page). For participants under 18, this form must be signed by a parent or legal guardian. Please fill out the form legibly and completely to expedite the check in process when you arrive at Talent. The "witness" line should be signed at Talent by the adult that is bringing the minor named on the waiver.
